GENERAL OFFICE POLICIES AND INFORMATION 

The Path to Optimal Health. The Future of Medicine.  

FUNCTIONAL MEDICINE CONSULTATION 

Prior to your initial visit with our practitioner, your completed health history and symptom  questionnaire forms, and all supporting data will have been reviewed. These are to be uploaded  to the portal at least 48 hours prior to your initial appointment. During your first encounter, you will have time to elaborate on your unique story. The consultation includes joint review of  all submitted information, an extensive conversation about past and current health (including  symptoms, lifestyle, diet, stressors, medications/supplements), a limited physical exam if  pertinent, and initial recommendations to start you on your journey to optimal health. 

In essence, you can expect a holistic examination of your overall health to be conducted by the  practitioner as described above. Symptoms will be linked, and common threads uncovered. You  will be afforded ample opportunity to ask questions and have your concerns addressed. This  appointment is typically one hour, however, please allow up to one hour and 15 minutes. 

A verbal summary will be provided of the key tenets at the conclusion of the first meeting. In  addition, each client will receive a written follow-up within 48 hours via CLW’s secure email or  patient portal. This communication will include the summary discussed along with personalized  recommendations for symptom relief and advancement toward feeling your absolute best! Accompanying materials will be provided as deemed suitable. 

FOLLOW-UP 

Since there is not a one-size-fits-all approach when it comes to functional medicine, it requires  time to successfully meet your needs and goals. If mutually decided that we are a good fit to  work together, bundled health care plans* are offered which include subsequent visits with high level follow-up and support for getting and staying on track to achieve optimal health. 

At CenterLine Wellness we provide each client personalized attention as we work together to  support your body in healing. There are not any protocols or algorithms since we recognize there  is not one mold that fits all. Each client’s story will be heard and a sense of partnership with a  trusted and experienced practitioner will be felt. 

While working together, you may be recommended lifestyle modifications, personalized  conventional and/or functional lab testing, stress reduction strategies, a personalized supplement  regimen, and/or nutritional plan. We strive to find and maintain the delicate balance of your  unique individual body and its interconnected systems. 

*Bundles are nonrefundable.

PAYMENT MODEL 

CenterLine Wellness, LLC is a cash-based* practice. While functional medicine is not  recognized by insurance companies in New York, many of them do offer out-of-network benefits  after a deductible is met. Upon request, a Superbill will be rendered for you to seek  reimbursement. We cannot guarantee reimbursement or that an insurance company will accept  the Superbill towards your deductible.  

*Cash, credit/debit cards, HSA and/or FSA cards are accepted forms of payment.  Local checks are accepted. There is a $50 fee for each returned check.  

You agree to pay any applicable surcharge on payments made by credit card. All amounts shown  on our website are in US dollars. The currency and amount will be displayed before you make  your payment. We do not store your financial information on our platform. Our third-party service provider, Square, will store your details and will use these details for each successive  billing period. We are not responsible for your data with Square. CANCELLATION POLICY 

If a client is unable to attend a session, cancellation at least 48 hours in advance is required. A  fee of $100 for the initial appointment and $50 for each follow-up visit will be applied to the  account for cancellation within the 48-hour window; this also applies to no-shows.  

COMMUNICATION 

We value your privacy. Our health portal, email, and virtual platform are HIPAA (Health  Insurance Portability and Accountability Act of 1996) compliant.  

Through our email system, you will receive confirmations, reminders, recommendations, and  information pertaining to functional health. You do have the option to opt out, however, we do  not recommend doing so since this will remove you from ALL notifications.
